If you don't have the Snapple rebate form, you can apply for the rebate without it!

One of my Facebook friends just told me that you do not have to have the form to get the Snapple 6 pack (up to $6.99) rebate (not valid in Rhode Island).  That is great because I was only able to find one form, so now I can submit the rest of my rebate requests. If you submitted for a previous Snapple mail-in rebate, you can also participate in this one. All you need to do is provide the following information on an index or post card

Name
Address
Phone and/or email address
Flavor purchase
Price paid
Store name

You will also need to include your original cash register receipt, dated between 3/27/11 and 6/30/11, as well as the UPC code found on the Snapple box. Submission should be sent to the following address and must be postmarked by July 11th.

You can request a rebate per person in your household (5 request maximum per household). If multiple Snapple products are purchased in the same transaction, one receipt will work as long as the receipt shows multiple purchases.
